Fournier gangrene: a case report

Abstract

Fournier's gangrene is a severe, rapidly progressive and necrotizing polymicrobial infection of the skin and soft tissues with anorectal and genitourinary focus. A 34-year-old male case is present, who presents with fever, pain and increased volume in the pelvic, scrotal and gluteal regions; 6 days prior to admission he presented fetid purulent secretion receiving Imipenem, Vancomycin and Metronidazole. He was subsequently taken to surgery where inguino-scrotal necrectomy, deep lavage, and colostomy were performed. The culture of the secretion reported Staphylococcus aureus and Escherichia coli sensitive to treatment, due to symptomatic improvement is discharged with outpatient treatment. The advent of new non-invasive therapeutic measures has not displaced timely radial treatment with surgical debridement, which warrants prolonged hospitalizations with an economic impact on the institution. The objective of the case is to make known the therapeutics used, as well as to reinforce the theoretical-practical knowledge of this infrequent and extremely aggressive infection.
